50forward transition to Help Direct

Background

The newly established Help Direct service was launched in November 2008. The service, commissioned through Lancashire County Council to a number of lead providers across Lancashire, is the new gateway providing one point of contact for adults to access information, advice and services. Signposts is sub-contracting with Age Concern Lancashire, the lead provider in the Lancaster District to deliver outreach information, advice and support.

The Navigator model developed through the 50Forward Project is being used by Signposts to deliver the outreach casework for Help Direct, offering low level intervention to all adults referred from the Help Direct team. In addition, we provide the same service under the Help Direct umbrella, to referrals received directly to Signposts from other agencies and via our own work with disadvantaged and hard to reach groups.

Help Direct is working with a large number of local organisations and community groups to provide and develop support for adults and older people to get the most out of life in the following areas:

  • Health and fitness
  • Home and Garden
  • Feeling safe in your home and community
  • Learning and leisure
  • Managing finances
  • Mobility and transport
  • Community groups and involvement
  • Getting the right help and support
  • Relationships and families
  • Employment, volunteering and training

Knowledge review 13: Outcomes-focused services for older people

This SCIE knowledge review includes a review of research evidence on the outcomes valued by older people and the factors that facilitate and inhibit achieving these outcomes.

It also contains a postal survey of localities and social services managers in England and Wales known to be interested in developing outcomes-focused approaches to older people’s services; and in-depth studies in six localities.
